From the First Referral to Recovery.
Surgical care can involve many decisions. Hinkapin Health brings them together through one coordinated navigation process.
From the moment a referral is received through recovery and follow-up, our team helps keep the patient informed, the care coordinated and the appropriate people connected.
The Hinkapin Journey
We Receive the Referral
A referral may come from an employer, health plan, TPA, benefits team, physician or the patient. We begin by understanding the reason for the referral and confirming the necessary information.
We Connect with the Patient
In most cases, we attempt to reach the patient the same day. A navigator explains the process, gathers relevant information and becomes the patient's primary point of contact.
We Gather the Medical Information
Our team helps obtain relevant medical records, imaging, physician notes and other information needed to understand the patient's condition.
We Understand the Clinical Need
We look at what the patient has already tried, what has been recommended and whether additional evaluation may be appropriate. When needed, we help coordinate a second opinion.
We Identify the Right Specialist
Based on the patient's diagnosis, location, medical needs and procedure, we help identify an appropriately qualified physician.
We Identify the Appropriate Site of Care
We consider the patient's clinical needs and procedure when coordinating an appropriate facility, whether a hospital, ambulatory surgery center or other suitable setting.
We Establish Clear Pricing
Whenever possible, the surgical episode is arranged at a predetermined bundled price, giving the patient and employer greater visibility into the expected cost before care is delivered.
We Coordinate the Surgery
Our navigation team helps coordinate scheduling, records, communication, preoperative requirements and other details needed to prepare for the procedure.
We Support Recovery
Our involvement continues after surgery. We help coordinate follow-up and recovery needs and make sure the patient knows where to turn with questions or concerns.
We Measure the Outcome
We track relevant measures such as patient satisfaction, complications, postoperative utilization, recovery, compliance, surgical outcomes and cost savings. These insights help us continually improve the program and give employers greater visibility into the value being delivered.
