Because every patient and every surgical procedure is unique, the final bundled price may vary based on medical necessity, patient-specific circumstances, and the scope of services requested.
1. What is included
Unless otherwise specified in writing, a bundled surgical price may include:
- Surgeon professional fee
- Ambulatory surgery center facility fee
- Anesthesia services
- Standard implants and surgical supplies (when applicable)
- Routine perioperative services
- Patient navigation and care coordination, when included in the program
The exact services included in each bundle will be identified before surgery.
2. What is not included
Unless specifically stated in writing, bundled pricing does not include:
- Hospital admissions
- Overnight hospitalization
- Emergency department services
- Intensive care services
- Treatment of complications
- Readmissions
- Return to the operating room
- Revision surgery
- Corrective procedures
- Repeat procedures
- Additional procedures performed because of unrelated medical conditions
- Services provided by physicians or facilities that are not participating in the approved surgical episode
- Pathology services
- Advanced imaging
- Laboratory testing not specifically included
- Prescription medications
- Durable medical equipment
- Physical therapy
- Home health services
- Travel, lodging, or transportation expenses
Additional medically necessary services may require separate authorization and separate payment.
3. Employer programs
For employer-sponsored bundled surgery programs, the exact scope of covered services is determined by the agreement between Hinkapin Health and the employer, third-party administrator (TPA), or benefit plan.
Individual employer programs may differ.
4. Insurance
Some Hinkapin Health programs are offered as self-pay or employer-sponsored bundled payment arrangements.
If a patient elects to use private insurance, Medicare, Medicaid, TRICARE, or another governmental healthcare program, bundled pricing may not apply.
Insurance coverage, deductibles, co-payments, co-insurance, and reimbursement are determined solely by the patient's health plan.
Hinkapin Health does not guarantee insurance coverage or reimbursement.
5. Medical outcomes
Medicine is not an exact science.
Although Hinkapin Health carefully selects experienced physicians and accredited facilities, no physician, surgery center, or healthcare organization can guarantee a particular medical outcome.
